My dad just underwent a AAA repair. The doctor knowingly placed the the graft stent to high with minimal encroachment on the Right renal artery and complete mechanical obstruction of the left renal artery. He told us that. He said that he knew he had placed it alittle high but since he noted sufficient blood flow via arterogram in surgery he felt that the everything was fine. But my dad was on heperin which thinned the blood so the it did appear that there was good blood flow. But when he coagulated the the blood was to thick to pass through the obstructions. My day was taken back to surgery 6 hours later because they found that he was becoming in distress and the Doctor pulled the graft down by 1 cm. My dad is now in Acute renal failure. He had the surgery on April 23, 2008. He now has had 2 dialysis treatments to pull fluid off of him because of low unrine output of about 60cc/hour and he had retained almost 7 liters of fluid with increasing bilateral infiltrates. I know this situation is a rare and atypical occurance. What's the prognosis in this sort of situation. He was compromised for 5 to 6 hours. The renal ultrasound right prior to the second surgery showed NO blood flow in the Left kidney and minimal on the Right kidney. The most recent renal ultrasound showed improving flow in the right but no doppler flow in the left but had some color flow noted. I know his left renal artery was lower then the right. So how could the doctor not think that if he minimally encroached the right renal artery as per OR report that he did just obstructed the left renal artery complety be way of simple anatomy? Which is why the left kidney took the biggest blow for level of damage. He said when he went in the second time he pulled the graft down by 1 cm. My fear is did he just pull it down so it was no longer encroaching on the right renal artery or did he pull it down far enough that the left renal artery was also cleared by the graft. How much distance is inbetween the 2 renal arteries?
Any advice on this would be great. My family is very confused, mad and upset at this unnecessary occurance. He said in his career this has never happened before. The only thing that I can find in the OR note to have contributed to this happening now is there was some "Flouro Movement" during deployment. But if he knew it was too high and he just obstructed the left renal artery why didn't he pull the graft down immediately?
