4% renal function question

My cousin's boyfriend had a blockage in his kidney which are stones which caused an obstruction. They put something in his kidney to drain it for I think 2 weeks. They then did a test to see how functioning his kidney is. The urologist mailed him a letter stating his left kidney is contributing to 96% of his overall renal function, while the right is only 4%. The urologist recommends removal of his kidney to prevent any infection getting up in to his kidney . My opinion is any doctor to mail you a letter instead of setting up an earlier doctors appointment is very unprofessional. Has anyone ever had this situation and went a different route? I suggested he go see a nephrologist since he specializes in kidney's. Does anyone think this is a bit extreme or standard procedures? Can they keep the kidney with only 4% function if they remove those kidney stones surgically?

Any input advice would be helpful she is worrying herself sick over her boyfriend and it just doesn't seem right to me that they didn't remove the blockage and THEN check his kidney function
