Hi, I'm no doctor, but some of what you wrote sounds a bit too familar to me.
First you need to be tested more before anyone jumps to conclusions. Have you had a renal panel done as a blood test? Have you ever been tested for autoimmune diseases, you might bring that up to your regular physcian and the kidney tests too. Or get a new doc who will look further into it for you. A test for urinary infection will only show if there is an infection or not. The pain you explain sounds all too familiar as far as it going into the shoulders, I have a tendancy to get or have prolonged pleuritis/pleurisy...not caused from kidney disease. I don't know if these suggestions will help ,but its a start.

I can not make any diagnosis over the internet nor is that the purpose of this site. You have multip[le complaintss and symptoms and you need a comprehensive evaluation by your doctor .From the kidney standpoint measuring the creatinein and BUn and electrolytes. Also checkingf hte urine for proteinuria seems reasonable. Good luck
