kidney disease

My husband has recently been told he has diabetic kidney disease
3 weeks ago he suffered a heart attack
he seems to have got over the worse, problem is he is now suffering lower back pain,difficulty going to the toilet, breathlessness, also he is tired all the time, i dont know if this is connected to his kidneys, or his heart attack
can anyone please help
