Dear Drs.

Thank you for your web site. I have found this most helpful.

I have several questions. If a urine cytology is negative, is it most likely that Ca is ruled out or are biopsies needed?

I have had mild right flank pain and ureteral pain near the bladder junction, with bladder spasm type symptoms for approximately 3 months. The bladder feels somewhat better when on antibiotics but I never seem to get rid of the ureteral pain. As soon as I come off the antibiotic the full blown symptoms occur again. I have passed what looks like shreds of tissue paper in the urine, which clears when on antibiotics. There is + to +++ hemolyzed blood in the urine on or off antiboitics. The CT is negative for stones. A urine cytology was run and negative. I am scheduled for a cystoscopy uteroscopy and retrograde pylogram. Is it possible to have a small stone that does not show on CT, but is causing reinfection? I have had a colonoscopy and all is well. Thanks for your help.

If a urine cytology is negative, is it less likely that Ca is ruled out, but biopsies might still be needed, depending on the individual clinical scenario.
It is possible to have a small stone that does not show on CT; typically the cysto/retrograde should make the correct diagnosis and lead to resolution of the problem.
